Plan employees may:

1. Disclose PHI to a Plan Sponsor in order to carry out administrative functions that the Plan Sponsor performs, but only according to the Policy regarding Uses and Disclosures to the Plan Sponsor
2. Not disclose PHI to the Plan Sponsor for the purpose of employment-related actions or decision or in connection with any other employee or benefit plan of the Plan Sponsor

Women’s Health and Cancer Rights Act Annual Notice

• All stages of reconstruction of the breast on which the mastectomy was performed;
• Surgery and reconstruction of the other breast to produce a symmetrical appearance;
• Prostheses; and
• Treatment of physical complications of the mastectomy,包括 lymphedema.
